I think I might have a Rotator Cuff Injury, will physio help?

Rotator Cuff Physio / Shoulder Physio:

At Somerton Physiotherapy we treat shoulder issues daily. Some of the more common conditions we treat include osteoarthritis, frozen shoulder, post-surgical repairs, and rotator cuff tendinitis/tears. Recently in clinic we saw a 42 year old man working in the construction sector complaining of left side shoulder pain over a number of weeks that was affecting his work and sleep. He really struggled with lifting any objects over his head, doing any jobs that involved reaching overhead, and in the past week had become painful in bed when lying on his left side. After a thorough assessment he was diagnosed with rotator cuff related shoulder pain (RCRSP).

 

What is RCRSP?

RCRSP is an umbrella term that encompasses a broad spectrum of shoulder pathologies including: Subacromial pain/impingement syndrome, rotator cuff tendinopathy, and symptomatic partial and full thickness rotator cuff tears. It is usually classified as diffuse pain over the deltoid/upper arm region that worsens with overhead or behind the back reaching. The rotator cuff muscles are a group of 4 muscles on the shoulder girdle that provide stability to the shoulder and are essential players in providing movement and strength in almost all shoulder movements.

>80% of shoulder pain is classed as rotator cuff related (Ostör et al., 2005), and the mechanism of injury is found to be multifactorial, with load on the rotator cuff tendons playing a primary role. Lack of strength and overuse of the rotator cuff muscles leads to overloading on one or more of the tendons surpassing its’ physiological limit. This leads to weakening within the structure of the tendon, pain, and subsequent structural failure.

 

As discussed above, the cause of RCRSP is multifactorial. Aside from load, there are some predisposing risk factors to RCRSP. Those with an increased risk of developing RCRSP include:

 

  • >50 years old
  • Females > Males
  • Diabetics
  • Diagnosed Cardiovascular Disease
  • Diagnosed Osteoarthritis
  • Overweight
  • Occupations that include loading with the shoulder elevated >90°
  • Smoking/Alcohol consumption
