What Are the Common Causes of Achilles Tendon Pain?
At Somerton Physiotherapy we regularly treat patients attending our clinics with heel pain / Achilles tendon pain. Heel pain / Achilles tendon pain can have various causes. A common cause of heel pain that we treat daily is Achilles related heel pain. Recently, a 24-year-old nurse attended our Castleknock clinic complaining of left heel pain at her Achilles tendon. She had recently started working full time after graduating from college and reported her heel pain started at the same time. The pain started as soon as she started working long shifts and being on her feet all day. After attending the clinic and a thorough assessment she was diagnosed with Achilles tendinopathy.
What is Achilles tendinopathy?
Achilles tendinopathy is a common overuse injury as a result of degeneration of the Achilles tendon due to repetitive overloading and microtrauma that leads to pain, swelling, and functional impairment (von Riceknbach et al., 2021).
What leads to Achilles tendon pain?
There are many internal and external factors that can lead to an Achilles tendon issues, such as an Achilles tendinopathy. These include:
- Overuse/Overtraining
- Ramping up activity/training too quickly
- > Age
- Male > Female
- Certain metabolic / Rheumatological disorders
- Overweight
- Overpronation / Flat feet
- Poor running technique
- Inadequate Footwear
- Certain medications
- Inadequate lower limb muscle strength
What can I do to help my Achilles pain?

Initially, you can RICE – Rest, Ice, Compression, Elevation. This will help reduce the pain and swelling associated with an Achilles tendon issue. Minimise the activities that increase your pain to limit any further damage. Ignoring the issue can lead to further damage to the tendon, such as an Achilles tendon rupture, which would require surgical intervention. The next step should be to contact an experienced physiotherapist to have the issue assessed and in order to put a plan in place towards recovery.
How can physiotherapy help my Achilles pain?
Following a physiotherapy assessment, you will have a better understanding of the cause of your heel pain, be it an Achilles tendinopathy, or another pathology of the Achilles. Once the cause of your pain has been identified your physiotherapist can provide advice and guidance on load management, and start your rehabilitation programme that will involve strengthening the Achilles tendon and the surrounding structures to get you back to the activities/sports that you enjoy.
